This is more simply a toy than a phrase, but where does it come from?
Well, there was a toy where something would spring up from the middle of a box and surprise someone; this toy was called a jack in the box.
So someone described as being like 'a jack in the box' is being compared directly to the toy, as someone who appears unexpectedly.
